If you download, print and complete the appropriate questionnaire before attending your initial appointment you will save yourself 10 mins in the waiting room. Be sure to bring the completed form with you for your first appointment at Southside Chiro & Massage. Alternatively, you may prefer to arrive approximately ten minutes before your scheduled appointment time and we will provide you with a questionnaire to complete at the clinic.
